Safety
Student safety is a high priority at CREATE. All students are issued picture ID’s; there are security cameras placed at the entrance, in the stairwells, in the classrooms, and in the cafeteria. No one may enter the building without the door being buzzed open. All visitors must sign in. There is a full-time security staff on duty whenever students are present, and after dismissal. Any individual that threatens student or staff safety will be dealt with to the extent necessary, and in accordance with the law. The school maintains a zero tolerance policy for violence.
Health
CREATE Charter High School has a full-time certified school nurse. If a student gets sick during the day, the nurse will determine if the student needs to be sent home. A parent or guardian must come to pick up any student.
When a student is ill, we ask that the parent sends a note or calls the nurse, and informs the school of any long term absence. Medical excuses from school or physical education will only be accepted when generated from a licensed medical doctor, doctor of osteopathy, doctor of dentistry, advanced practice nurse or physician’s assistant. Excuses must include diagnosis and anticipated length of exclusion.
If a student needs to be excused from physical education classes or has restrictions on the amount of activity, the nurse should be informed and will forward the information to the teachers. A doctor’s note will be required. The doctor must then clear the student before full participation in physical activities can resume. Likewise, please notify the school nurse of any illnesses or conditions that would interfere with the student’s participation in normal classroom activities. If a student has to use the bathroom more than usual due to an existing medical condition, please notify the nurse and provide appropriate documentation.
The school nurse will maintain all health records. All students must have up-to-date immunizations as required by State law and code. If immunizations are not up-to-date, the nurse will inform the parent or guardian of the missing immunizations. Students that do not have the appropriate and required immunizations may not attend school.
If an injury or illness occurs during school hours, students will be triaged by the school nurse and appropriate first aid will be administered. If it is determined that the student needs immediate medical attention, 9-1-1 and the parent/guardian will be notified. If it is determined that the student should receive follow up care by their private physician but are not in need of emergency care, the parent will be informed.
All health records are confidential, legal documents. Access to these records is restricted to the Certified School Nurse and School Physician.
Any and all medical conditions may not be disclosed to any teacher or staff person without written permission from the parent/guardian. If their teacher is informed about a condition, diagnoses are never discussed. The teacher can be advised of symptoms, restrictions or special considerations for that student with parental consent.
Exclusion from School – Illness related
Any student with a known communicable disease (i.e., flu, TB, chicken pox, etc), shall be excluded from attending school until a doctor’s note is received clearing the student. If a student becomes ill while in school, the parent/guardian will be notified by the school nurse and asked to remove the student from school if the student has a fever of <101 F. , vomiting, diarrhea or exceptional complaints. The Certified School Nurse is to determine sufficient medical reasons for exclusion from school.
Medical Incident Report
Any and all injuries sustained by students while in the charge of C.R.E.A.T.E. Charter High School will be reported via Medical Incident Report, and will be sent home with the student. In the case of serious or life threatening injuries, the parent will be called immediately, as will the emergency response team (9-1-1).
Athletic Participation
No student shall be allowed to participate in any extra-curricular physical activities (including practices) without prior clearance from the student’s private physician or C.R.E.A.T.E. Charter High School’s school physician. The Athletic Pre-Participation Physical Examination Form (parts A & B) from the State of NJ DOE/NJSIAA must be completed prior to a student’s participation. Extra-curricular physical activities include any school athletic squad or team; whether competing against other schools or not.
Medication in School
Medications can only be dispensed by the Certified School Nurse as per NJDOE-Health Services guidelines. Students may not carry medications on their person. The only exception to this regulation is if the student has asthma or severe allergies and a doctor has completed the form for self-administration of inhalant drugs and epi-pen. In that case, students may have these medications on their person. All other medications must be dispensed by the Certified School Nurse and require a physicians order. Teachers/staff members are never to dispense medications.
Suspicion of Student Being Under the Influence of Drugs and/or Alcohol
If a teacher, staff member or administration believes a student may be under the influence of drugs/alcohol, the student should be assessed by the Certified School Nurse.If the Certified School Nurse, after assessment of pupil reaction, coordination and level of consciousness, suspects intoxication, the following steps shall be taken:
Parent/guardian of student must be called into school.
Giant Steps (Franciscan Health Center), 179 Palisades Ave., Jersey City, 201-795-5805 will be contacted by the school nurse.
A physical examination will be conducted by a physician.
A 401 form will be issued: “rule out drug/etoh use, may student return to school?”
Parents must transport student to program for drug/alcohol screening. The certified school nurse will follow up with treatment plans with the parent and Giant Steps.
Child Abuse Reporting
School personnel – including professional staff members, support staff members, and unpaid volunteers – who have reasonable cause to believe that a child has been subjected to child abuse or neglect or acts of child abuse or neglect as defined under NJSA 9:6-8.9 must by law immediately report to the Division of Youth and Family Services. The suspicion of child abuse and/or neglect may be based on the complaints of the student, on the direct observations of the employee over a period of time or both.
Although abuse is more easily recognizable, neglect can take more subtle forms. Neglect includes failure of the caregiver to provide food, shelter, adequate supervision or medical care. Failure to send a child to school and to keep a child reasonably clean may also be considered neglect.
School districts are mandated to cooperate with DYFS. DYFS caseworkers may interview a child at school as long as they present proper identification, but only in the presence of the school principal or designee. The purpose of the school representative is to provide support and comfort to the child, not to participate in the investigation.
School Districts are mandated to provide annual delivery of information and in-service training to school personnel concerning:
child abuse or neglect instructional methods and techniques relative to issues of child abuse or neglect in the local curriculum
personnel responsibilities pursuant to NJSA 9:6-8.10 et seq.
All new school district employees, both paid and voluntary, must receive the required information and training as part of their orientation.
